【講義目的】
生活の場で必要とされる療養者とその家族に対する看護の展開と、多職種と協働して地域包括ケアを提供する力を養う。
【到達目標】
1.在宅療養者とその家族の、健康問題や生活環境等、個別性に応じた看護の展開の特徴を説明できる。
2.地域包括ケアにおける多職種連携について理解し、そこで期待される看護師の役割を説明できる。

生成AIの利用にあたっては「本学の教育における生成AIの取り扱いについて（学生向け）」の記載内容について留意すること。
この授業においては、教員の指示する範囲において生成AIの利用を許可する。これ以外の範囲での利用は禁止する。生成AIの出力した内容について、事実関係の確認や出典・参考文献を確認・追記することが重要である。また、生成AIによる出力結果をそのままプレゼンテーション資料として提出してはならない。

授業中に指示した宿題や事前・事後学習はもとより、「講義内容・授業計画」に記載したテキスト等の該当箇所などについて、十分な予習・復習をして講義に出席すること。（0.5ｈ×８週＝4ｈ）

事前課題で各自が調べてきたことをもとに、授業の中でグループワークやディスカッションを行う。

Grading Criteria and Methods

【成績評価の基準】
　在宅看護における看護の展開方法、在宅看護活動に必要な基本的知識、および、地域包括ケアにおいて看護師が果たす役割を理解できる者に到達度に応じて成績を付与し、S（90点以上）、A（80〜89点）、B（70〜79点）、C（60〜69点）のものに単位を与える。
【成績評価の方法】
　看護過程の展開の演習課題（20％）、および、定期試験（80％）で評価する。

How to Disclose Assignments and Exam Results

グループワークに関しては、授業の中で講評を行う。定期試験については、全体的な講評をユニバーサルパスポートを通じてフィードバックする。

Precautions and Requirements for Course Registration

Practical Education

訪問看護の経験のある教員が在宅療養の場で看護援助を提供した実務経験と専門的知識を通して講義することから、実践的教育に該当する。
